From review of a previous post, this fault seems to be indicating a problem with io module 3, which is an analog input. Is there anyway to test this or do I just have to swap in another module? Is there anything else to check?

Enrique, Thanks for the guidance. Question if we have high voltage anyway at this site - 240 instead of 208, that can cause the bus voltage to rise as well correct? The supplier of this equipment recomends installing transformers to step down to 208. They said the higher voltage can result in faults ...

We have a powerflex 4 controlling a 3/4 hp motor connected to a gearbox. The motion/position is controlled by a set of proxmity switches. Assumeing we have checked/verified all the issues potentially associated with the sensors/wiring, is it possible the drive could be 'extending' the length of the decleration? ...
