Update: After further troubleshooting and investigating we found that the power supply for the second expansion rack was undersized causing the fault to occur. We replaced it with a P4. We replaced the power supply in the primary rack but not the expansion rack.

Thanks for the very informative response Philip. A couple weeks go we indeed did put a power monitor on the power supply for 48 hours and observed no abnormalities with that. We do not use the 24VDC on the P4 Power Supply nor did we on the P2 that we had the same issue with both installed. ...

I was starting to consider the cable, i ensured the connection was seated properly when i swapped the rack out but the cable itself has indeed not been replaced, ...
